Forms and Resources

The following forms are required in order to live in the residence halls.

Health History Form -- This form must be completed online prior to your move in date. Failure to have this form submitted will mean you will not be allowed to check into the residence halls.

Missing Person Form -- The Missing Person Form is also a requirement by the Higher Education Opportunity Act of 2008. Please fill this form out online prior to move-in.

Residence Hall Packing List -- This is a list of items that you should bring to help set up and personalize your room for the school year.

Residence Life Handbook -- Represents all policies and procedures that apply to residence hall students. All residents are responsible for knowing and following policies outlined in the handbook.

Microfridge Rental Information -- This is a flyer from our vendor who rents the Microfridge - a one unit refrigerator, freezer, and microwave combo.

Student Renters Insurance -- National Student Services Inc. is a company who provides renters insurance to insure belongings in residence hall rooms.
